Social Worker / Social Services Coordinator

The Friendship Home is a 60+bed, non-profit long-term care community, currently undergoing extensive renovation. We are a mission-driven organization with a strong reputation, a home-like resident-centered care model, great survey history, and are blessed with longevity of dedicated staff.
Due to pending retirement, we are searching for a compassionate and outgoing Social Worker / Social Services Coordinator.
Working closely with the Interdisciplinary Team, this position is responsible for planning, developing, organizing, implementing, evaluating, and directing the social service program of the facility. Exhibits excellent and empathetic communication style with residents, families, co-workers, and the community. Responsibilities include but are not limited to observing, assessing and reporting resident condition changes, collecting data relevant to resident assessments, Admission and Discharge planning, use of and referral to community resources, Behavioral Health monitoring, helping to arrange resident transportation, assisting with psycho social needs of the residents, participating in Quality Assurance and Performance Improvement initiatives, and assisting with public relations and marketing.
Candidate must have excellent communication skills, have a strong passion for resident-centered care, and be team oriented. Familiarity with MDS, PASSARs and Matrix software a plus.
Education/Experience
Associate degree from an accredited college or university. A minimum of 3 years experience in a nursing care facility or other related medical facility preferred. Must have strong working knowledge of Medicaid/Medicare/Managed Care processes, as well as Veteran’s assistance benefits. Preference may be given to licensed Social Workers.
